Summary Case Report: 27-year-old self-employed trucker was cleaning the inside of his tanker truck, which was normally filled with liquid asphalt. He was using a mixture of gasoline and diesel fuel, which was the normal procedure. He was working alone, and was not wearing a respirator. He was overcome by the fumes and was asphyxiated while in the tanker. The man was found by his brother who called 911 for rescue.

Summary Case Report: 40 year old laborer at a corrugated paper manufacturing plant was working at a winder station, helping two other employees thread paper onto the winder machine when a large metal plate fell on top of the victim, pinning him against a beam. He was crushed and died of asphyxiation. The machine did not malfunction.

Summary Case Report: 15-year-old Ecuadorian maid was found stabbed to death in an inner- city hotel. She was not sexually assaulted. Speculation was that she was in the country with falsified documents. Police have no leads in the murder, although a similar case in the same city a few months earlier. Managers at the hotel have changed housekeeping procedures, which now have maids working in pairs as they clean.

Summary Case Report: 48 year old paraplegic (lower extremities) was working on his farm doing tiling work when he was involved in a motor vehicle accident in his pick-up truck. He received spinal injuries (unspecified), and was paralyzed in his lower extremities. He never returned to farming, but spent most of his time simply sitting in a wheelchair. He suffered a pulmonary embolism and died, indirectly from the MVA years earlier.

Summary Case Report: 48-year-old seed salesman had purchased a small grain elevator, and was towing it behind his truck. While backing out of a driveway, he turned too sharp, wheel went over a snowbank, and it overturned in a street. Several men tried to lift the elevator, assisted by a nylon clothesline attached from a car to the elevator. When it was in the air, the rope broke, and the machine fell hitting the man in the head. Summary Case Report: 29-year-old waitress for a 24-hour restaurant was working the night shift. She was found stabbed to death in the men's rest room about 2:30 AM. The case register had been robbed and her car had been stolen. Police found her car a few blocks away and arrested a suspect, but little more is known. A cook was also working at the time, but he did not hear/see anything unusual. She had worked there for 8 months.

Summary Case Report: 68 year old station manager at a grain elevator was helping to unload dry corn out of a bin using the bottom unloader auger. He was found missing, and others thought he might be trying to unclog this auger. He was found totally submerged in the grain, dead from suffocation. He was not caught in the auger or even near it. For an unknown reason, he entered bin and was caught in the flowing grain.

Summary Case Report: 53-year-old road worker for the DOT was part of a crew sealing cracks on an interstate highway. A semi truck entered the work zone crashing into a string of three DOT trucks parked on the roadway. The victim was walking on the roadway and was crushed between two trucks and died at the scene. Another DOT worker, in one of the trucks, died later in the hospital. The semi veered into the median and caught fire.

Summary Case Report: 41-year-old truck driver for the DOT was part of a crew sealing cracks on an interstate highway. A semi entered the DOT work zone and crashed into a string of three DOT trucks parked on the highway. The victim was sitting in the first truck that was hit by the semi. He received severe injuries, and died six days later. Another worker was crushed to death between two trucks as he worked on the roadway.

Summary Case Report: 36 year old farmer was driving his pickup truck down a county road when he attempted to cross a bridge at the same time as a garbage truck was crossing from the opposite direction. For unknown reasons, the garbage truck suddenly veered into the opposite lane and struck the pickup head on. The farmer was killed at the scene. His 5-year-old son, seated next to him, was treated and released from the hospital.

Summary Case Report: 61-year-old woman was working part-time as a maid for her neighbor. She was working in a new addition for the house, on the lower level. When she plugged in the vacuum cleaner, the house blew up. She was severely burned and died 2 weeks later. There was a faulty propane connection under the addition, and the ground and lower level were saturated with gas. The soil had absorbed the odorant (mercapten) in the propane and there was no warning.

Summary Case Report: 55 year old owner of a heating + plumbing company was installing a central air conditioning unit on the roof of a high school. His left hand made contact with a 440 volt electrical source and he was killed at the scene from electrocution. Autopsy revealed exit wounds on middle of back and burns on center of chest.

Summary Case Report: 64 year old owner of a grain vacuum service was helping to unload a 10,000 bu. bin, half full of dry corn. He was in the bin standing on the corn, while using a 6-inch suction tube to vacuum the corn. The bin auger was malfunctioning, so they were unloading it with a vacuum. The man was discovered missing, and eventually located submerged under the corn, dead from suffocation. Autopsy ruled out cardiac causation but could not determine if the man fainted or had other loss of consciousness. Full Report with Photos

Summary Case Report: 31-year-old worker for a house-moving company was part of a 3-man crew moving a single story house. They used metal railroad rails, welded together, as supports under the house. Victim went under house to get some tools and put his hand on one rail, and was electrocuted. The rail had contacted a power line to the house, which had never been disconnected. Co-worker used a board to move victim's hand off rail.

Summary Case Report: 52-year-old accountant and farmer was using an older narrow-front tractor to move dirt, filling in a ravine. He was traveling across the side of a hill with a load of dirt in his raised bucket when the tractor overturned pinning him underneath one of the rear tires. He was described as an overly cautious worker, and relatives suspect the load may have shifted. The tractor did not have a ROPS (roll over protective structure).

Summary Case Report: 70-year-old farmer was mowing weeds with a large utility tractor and bushog. He ran out of diesel fuel and his tractor stopped. The farmer refueled tractor and had to bleed air out of the lines. He had his granddaughter push in the clutch so he could turn over the engine. This was necessary due to safety switch. When tractor started the girl let out the clutch and the tractor (still in gear) ran over the victim.

Summary Case Report: 42-year-old worker for a large farm was helping to unload grain from a bin filled with 8 feet of corn. A bottom auger was running. When grain stopped flowing, the victim and a co-worker went inside the bin and began probing the grain to break up a suspected grain clump or bridge. The grain caved in burying the victim. The other man barely pulled himself free and ran to shut off auger. Victim died of suffocation.

Summary Case Report: 35 tear old Terex truckdriver for a crushed limestone quarry was dumping his truck at a 20-foot high waste dump. Berms or bumpers were not at the site and the victim backed over the edge, overturning the truck onto the driver's side. The victim was not wearing a seat belt and was ejected during the rollover and pinned under the left front tire. He was dead from massive crush injuries.

Summary Case Report: 38-year-old construction equipment operator was doing a welding job for a friend, making parts for a garbage truck. He was arc welding at the rear of the truck, using a 55 gal. barrel as a worktable. He accidentally burnt through the lid and the barrel exploded killing him instantly. There was a secondary fire that ensued. The contents of the barrel remain unknown, probably residual diesel fuel or gasoline.

Summary Case Report: 63 year old dredge operator with 7 years experience was found dead lying in 3 ft. of water next to a rock crusher machine for a cement plant. He died from multiple trauma after being caught in the drive belts of the machinery. He was apparently cleaning debris from the machine. By reports, he was not feeling well that day, but autopsy was negative. Belts guards had been removed and were lying nearby.

Summary Case Report: 42 year old water truck driver for an open pit clay mining company was driving his tractor pulling a 5000 gallon tank watering down a haul road. He failed to make a curve and the rig jack-knifed pushing the tractor into the tank. This crushed a fender, pushing the FOPS forward pinning the man to the steering wheel and forcing his head through the windshield. The victim had eight years mining experience.

He apparently slipped and fell hitting his head on a metal pipe on the ground. He suffered massive skull fractures and died at the scene. Autopsy could not rule out or confirm natural causes. His old combine did not have sufficient handholds or guardrails around the engine work area.

Summary Case Report: 60-year-old semi truck driver hauling a full load of grain was traveling down a gravel road. He crossed a railroad and was hit in the rear duels by a freight train traveling 50 mph. The crossing had warning signs, but no lights or crossing arms. The victim said his view was obstructed by a building. He suffered multiple injuries and broken ribs, was in stable condition, but died six days later.

The grain box was poorly attached to the running gear, and while unloading the grain, the center of gravity shifted causing the wagon to fall to the side crushing and asphyxiating the woman underneath.

Summary Case Report: 47 year old electrician was working from an aluminum extension ladder leaning against a steel building, replacing an emergency light system at a city landfill. The operating voltage of the light was 277 volts. The base of the fixture was open and multiple wires were exposed. Summary Case Report: 22-year-old self-employed tree trimmer was working at a private home topping out a tree. An assistant was operating a truck tied to the limb the victim was cutting. Suddenly the limb broke off and fell, and the victim fell also, receiving fatal head injuries when he hit the ground. The branch did not land on the victim. He was wearing a safety harness and tree-climbing spikes. Exact cause of accident is unknown.

Summary Case Report: 42 year old owner of an auto mechanic shop, along with one of his employees, was found crushed to death under a tow truck that they were repairing. The men used the hydraulic lift to raise the rear end of the tow truck, then put a jack stand under to support it. They then took off the rear wheels which defeated the parking brake, making the truck very unsteady. It simply fell off the jack stand while the men were working underneath. The "stinger" of the tow truck was in a slightly inclined position, which made the jack stand easily slip out of position.

Summary Case Report: 48-year-old farmer was loading grain into a semi-truck in a field driveway on his farm. The semi driver was positioning his truck, and the farmer was moving his tractor and wagon when he slipped off the top step of his tractor and fell backwards on the ground, directly in front of the dual tires of the semi tractor. He was partially run over and sustained fatal chest injuries. The top step was actually the cover for a battery case, in poor alignment with other tractor steps. It had been raining and was muddy.
