Participation in the survey is currently limited to Line 1 or "Q" cleared workers from 1945-1975 who were in some way employed by the Atomic Energy Commission now known as the Department of Energy.

If you are a former worker who would like to be part of the study, or if you are the
next of kin and you would like your deceased spouse or relative to be part of the
study, please call 1-866-282-5818 with the following information:

  • Name of worker, including middle initial. Please spell the name.
  • Address (to send the survey to)
  • Worker’s Social Security Number
  • Phone number
  • The years you or spouse/relative worked on Line 1, and job title (if known).
  • Health concerns/history.
