How to Apply
Deadlines for the Department of Biostatistics
- Fall Admission for MS or PhD
- January 15 (priority deadline for consideration for financial
support)
March 15 (final deadline)
If all required application materials have not been received by
the deadline, there may not be enough time to review your application.
Note that we do not consider applications for the Spring or Summer
semesters.
The Following Materials Should be Sent to the Admissions Office
- Application for Graduate Admission (apply
on-line at the University of Iowa Admissions website).
The program code for Biostatistics is 171.
- Application Fee (pay with credit card when submitting application)
- Official Score Reports from the GRE
- Official TOEFL
score reports, if required. If you are an international student,
you may contact the Office of International Admissions at (319)
335-1534 (115 Calvin Hall) to determine whether you are required
to take the TOEFL.
- Official Transcripts from all colleges and universities attended
mailed to: Office of Admissions, 107 Calvin Hall, University of
Iowa, Iowa City IA 52242-1396.

- Three letters of reference (no special form is required)
- Statement of Purpose (one or two paragraphs describing your
career goals and your interest in the program)
- Résumé describing educational and employment history
Please Note
All GRE
& TOEFL
scores must be submitted to the Office of Admissions by Educational
Testing Service (ETS). The University of Iowa's reporting code is
6681. The departmental codes for Biostatistics are 0204 for GRE;
59 for TOEFL.
We will not accept photocopies of score reports sent in by the
student as a substitute for the official scores. The College of
Public Health requires the GRE
general test. GMAT,
MCAT,
and LSAT scores
cannot be used instead of the GRE.
You will need to submit two official transcripts, one to the Office
of Admissions and one to the Department of Biostatistics.
There is no separate form to apply for financial assistance. All
applications received by the Department of Biostatistics are automatically
considered also to be application for financial assistance from
the department.
