Summary Case Report: 17 -year-old high school student was working at a neighbor's dairy farm helping with routine chores. He was assisting the farmer grinding feed using a tractor and PTO-driven feed grinder / mixer, which was old and in poor condition, without PTO or flywheel shields. The boy got too close to the unguarded drive-pin for the flywheel, and it snagged his jacket, pulling him into the PTO shaft, killing him instantly.

Summary Case Report: 51 year old owner of a small sand blasting company was trying to move a trailer-mounted sand blaster pot containing three tons of sand. He was backing up his narrow-front tractor, pulling the pot up a slight hill with a chain attached to front bucket of tractor. Needing more pull, he hooked a truck to the drawbar of the tractor with a chain. His wife was driving the truck (at an angle) down a driveway. While they were pulling, the tractor was pulled over sideways, overturning on the man.

Summary Case Report: 23 year old worker at a hog farm was power washing the hog buildings late at night. He removed the cover off an outside manure pit to check the level of manure. It was a cold night with freezing rain, and he apparently slipped on some ice and fell into the pit opening, hitting his head as he fell. He was found floating in the manure which was only four feet deep. Depth of the pit was eight feet.

Summary Case Report: 66 year old farmer was cleaning his barn with a skid-steer loader. He leaned out of the loader to shut a small gate, and he accidentally pushed a control lever bringing the lift arms down, pinning him to the machine. The mechanical seat-actuated hydraulic interlock was working but slight movement of the control beyond neutral made the interlock ineffective. The control moved freely and the bucket dropped on him.

Summary Case Report: 59 year old farmer was attempting to move a bag of soybean seed which contained 50 bushels. One bag was stacked on top of another bag in a barn. Using his forklift, he accidentally punctured the bag on the bottom. While trying to stop seed from running out of this bag, the grain in the top bag shifted and the top bag fell on him, pinning him to the ground. He died from mechanical suffocation.

Summary Case Report: 25 year old female laborer at a food production plant was cleaning out a tote station (4'x6'x4') when the tote frame became activated and came down on the woman, crushing her underneath. The air-controlled cylinder mechanism apparently had no lock-out, tag-out function, and was activated mistakenly when the victim was in the tote frame area.

Summary Case Report: 26 year old operator of a 3-wheeled agricultural sprayer was killed when his machine crashed through a barbed wire fence and careened down a steep ravine and overturned at the bottom. The machine had no seatbelt nor ROPS, so the man was thrown through the window and crushed as the cab crumpled in the overturn. He had just finished spraying an irregularly shaped field.

Summary Case Report: 47 year old farmer was planting corn with his father. An 8-row planter broke down in the field when the bolts attaching the tongue to the frame of the planter broke off. They managed to tow the planter to the farm intact. The victim then crawled underneath the planter to check the damage, and the tongue folded in two as the front of the planter fell to the ground. The farmer was crushed to death underneath.

Summary Case Report: 63 year old semi-truck driver was driving down a 2-lane highway, when he encountered another semi-truck being pulled out of the right-hand ditch. He swerved to the left slightly to allow room, but lost control due to his high speed, and collided with a third semi-truck traveling in the opposite lane. The two trucks crashed head-on and the victim was killed instantly from massive head injuries.

Summary Case Report: 36 year old farmer was checking his corn field while driving an all-terrain-vehicle (ATV). He hit a 10-inch terrace with his ATV, and it became airborne for a short distance, landed nose first in the dirt, then flipped over on the victim, causing fatal injuries. The man was working alone, and was found dead by a neighbor. The ATV did not malfunction.

Summary Case Report: 48 year old farm worker was helping a friend clear a field of rocks. Victim was driving a wide-front tractor with an attached front-end loader. The raised bucket was partially filled with rocks while the tractor was moving across sloping ground. Suddenly due to the imbalance of the rocks, the tractor overturned to its side, killing the operator. The tractor turned 360 degrees and continued downhill. There was no ROPS on the tractor.

He was traveling sideways on a very steep hill, when the rear wheels slipped on the wet grass causing the tractor to roll to the side. The farmer was crushed to death under the tractor as it rolled. The farmer had been using the same equipment, doing the same spraying job for several years.

Summary Case Report: 51 year old worker for a house moving company was working under a mobile home, disconnecting the water and sewer lines, preparing it for transport. Two other employees were on the opposite end of the trailer putting on the wheels. Suddenly the mobile home tipped toward the victim and fell pinning him to the ground. A large forklift was used to lift the mobile home off the victim, but he died later in the hospital from his injuries.

He attempted to avoid collision, but slammed into the rear of the semi. He was killed instantly. A fire developed and consumed both trailers. The other driver in the front semi was very inexperienced and was charged with several traffic violations.

Summary Case Report: 53 year old production line worker for an extruded rubber products company was killed while trying to fight a fire at the plant. A fire began at the end of a salt bath oven, and the victim (a retired fire fighter) was trying to put it out. Suddenly a pipe carrying molten salt exploded covering the victim with hot liquid salt. The man died enroute to the hospital. It appears the fire was caused by oil fumes from the salt bath.

Summary Case Report: 63 year old operator of a street sweeper was cleaning a highway for a construction company, which was working on the road at night, using auxiliary lights. He was hit from behind by a car traveling on the highway and killed instantly. By report the lights of the sweeper were not on since the driver could see using available light. Summary Case Report: 70 year old farmer was driving a newer model tractor down a state highway when he was hit from the rear by a delivery van. The driver of the van crested a hill, saw the tractor, yet could not stop in time, hitting the tractor in the left rear wheel. The farmer was not wearing a seat belt and was thrown from the tractor. He died a few days later while in the hospital. A slow moving decal was displayed properly.

Summary Case Report: 81 year old farmer was using a small utility tractor to move equipment on his land when he backed the tractor down an embankment causing the tractor to roll over. The farmer was killed when a hydraulic valve on the tractor crushed his head. The tractor had a front-end loader with a chain attached to sections of a harrow the farmer was moving. The tractor did not have a ROPS.

One steer knocked down the victim and either stepped or stomped on him in the chest area. The man crawled under a gate to escape, then went into the house to clean up. They were in the front engine when they crashed into the rear of another freight train on the same track. They were traveling 22 mph. Summary Case Report: 48 year old driver of a re-fueling truck for a highway construction company was preparing to turn left into a construction zone when he was rear-ended by a semi trailer carrying grain. Summary Case Report: 29 year old city maintenance worker was one of two men using a high-pressure jet/vac system to clean/flush residential sewers. After finishing a segment of sewer pipe, somehow the nozzle tip, weighing 10-15 lbs., came flying up out of the manhole and struck the employee in the face. The victim received severe head injuries and died two days later. The event was not witnessed and it remains unclear as to what exactly happened.

He was apparently trying to by-pass the safety switch with one hand, while the other hand turned the ignition key. He assumed the tractor was in neutral; however it was in gear. Summary Case Report: 56 year old agricultural tiling contractor was driving a tiling machine from one farm field to another. He made a left turn onto an asphalt road, and was going too fast or misjudged the turn, went off the right edge of the road (no shoulder) and rolled the machine in the ditch. He was pinned under the machine by his legs, conscious and alert. Deisel fuel was leaking and within a few minutes caught the machine on fire. The man died from the flames.

Summary Case Report: 42 year old laborer for a construction company was working under a church building which was setting on I-beams, in process of being moved. He crawled on top of an I-beam and was electrocuted. Co-workers had difficulty moving him off of the beam because of electric shock. CPR was attempted, but was unsuccessful. A live 110-volt drop cord was laying across the I-beam, and it had an exposed area of wire for 1/8 inch. It is likely this was the source of the electric current. The area around the beam and on the ground was wet.

Summary Case Report: 29 year old laborer for a limestone mining company was on his second day of work, driving a rubber-tired forklift/loader unit down a 19 foot-wide sloping road in the mine. He lost control of the unit and wedged it against a rib of the mine. Summary Case Report: 35 year old warehouseman was helping another man unload steel materials from a steel storage rack. There were very heavy boiler plates on top of the rack. Using an overhead crane they removed old steel pipes from the top rack, and the entire rack suddenly collapsed crushing the victim. Apparently over time the pipes provided lateral stability for the rack, and when they were removed, the storage rack collapsed under the weight of the boiler plates. .

A reciprocating jigsaw (sawzall) was on the roof and it started to slide down the roof. Summary Case Report: 68 year old dairy farmer was repairing shingles on the roof of his barn. He had tied a yellow nylon rope around himself with the end tied off to his pickup truck. He was standing next to a flatbed railcar while another man operating an excavator was moving a piece of wreakage. The excavator bucket was holding up a portion of a sawed off truck, including the rear dual wheels / axles. The load was being swung towards the railway car when the excavator operator noticed the victim standing near the railway car, and suddenly stopped his swing. The movement caused the set of wheels to come loose and they rolled towards the man and pinned him against the railway car.

He did not stop at the crossing but drove through at 15 mph. and was struck on the passenger side of his cab by an AMTRACK train traveling 79 mph. His semi seemed to explode into several pieces and the operator was killed from multiple injuries. Limited visibility may have been from a tree line and the fact the crossing was not at right angles to the road. They were unable to determine if he was wearing a seatbelt.

Summary Case Report: 37 year old maintenence engineer mechanic was working on a disabled forklift at a wire plant. He had jacked up the rear of the forklift and put some support blocks underneath, and then crawled underneath to work. The forklift apparently shifted and rolled off the support blocks crushing the man underneath.
