The goal of this presentation is to discuss the origin, evolution, and control strategies for H5N1 influenza viruses. Avian Pathogens as Zoonotic Public Health Threats

 

Prion Disease Research at the National Animal Disease Center

Prion diseases or transmissible spongiform encephalopathies (TSEs) are fatal neurologic conditions of animals and humans. Animal TSEs include scrapie of sheep and goats; chronic wasting disease (CWD) of cervids, such as deer, moose, and elk; transmissible mink encephalopathy (TME); and bovine spongiform encephalopathy (BSE). Since 1990, scientists at the National Animal Disease Center (NADC) in Ames, Iowa, have studied cross-species transmissions of the scrapie, CWD, or TME agent into livestock and wildlife in an effort to identify the potential host range of TSEs and their clinicopathological and molecular features in a novel animal species. In this presentation, Juergen A. Richt, D.V.M., Ph.D., lead scientist and Veterinary Medical Officer at the National Animal Disease Center in Ames, Iowa, will address the various TSE cross-species transmission studies underway at the NADC and the molecular characterization of BSE cases identified in the U.S.

The Emerging Pathogen and Bioterrorism Threats of Pox Viruses

Both humans and animals are susceptible to infections caused by the poxvirus family of viruses. Smallpox (variola), monkeypox, vaccinia, and cowpox are examples orthopoxviruses. Naturally occurring smallpox was eradicated worldwide in the late 1970s. However, smallpox remains a concern as a potential agent of bioterrorism since it is highly contagious, a large proportion of the population has no immunity, and there is little available vaccine. Emerging infections such as monkeypox also pose a concern to human health. In this presentation, Inger K. Damon, M.D., Ph.D., acting chief of the Poxvirus and Rabies Branch and chief of the Poxvirus Program in the Division of Viral and Rickettsial Diseases at the Centers for Disease Control and Prevention, will address these and other emerging pathogen and bioterrorism threats of poxviruses.

Advances in molecular epidemiology have dramatically enhanced our ability to detect and fix problems in our food and water supplies that formerly would have gone unnoticed. Recent outbreaks of E. coli O157:H7 and Salmonella spp. associated with fresh produce are good examples. In spite of these advances, the burden of foodborne and diarrheal disease remains substantial, both nationally and globally. John Besser, Manager of the Clinical Laboratory at the Minnesota Department of Health, will discuss and focus on systems used to detect and investigate problems in the food supply.

2006 Grand Rounds

Recombinant Reagents for the Detection of Biothreat Agents

Immunodiagnostics is the standard against which many agent detection, identification, and diagnostic technologies are compared. Antibody-based assays continue to serve as preliminary and confirmatory diagnostic formats for many infectious and non-infectious diseases. The increasing need for consistently high-quality immunodiagnostic reagents for biothreat agents and the difficulty of producing sufficient quantities safely in biocontainment makes recombinant antigens and antibodies the ideal solution. The ultimate goal of this scientific effort is to provide not only high-quality reagents for existing assays, but to develop a system that can quickly produce reagents to new and emerging biothreat agents. This program is presented by Dr. Randal Schoepp, chief of the Applied Diagnostics Branch in the Diagnostic Systems Division, at the U.S. Army Medical Research Institute of Infectious Diseases (USAMRIID).

Natural Toxins and Bioterrorism

In the current environment of political change and military conflict around the world, there is great concern that terrorist organizations may eschew direct military conflict and instead turn to nontraditional attacks on military or civilian targets. These nontraditional attacks may include biological agents such as bacteria, viruses, or natural or bioengineered toxins. This presentation will focus on the reality of toxins as agents of biowarfare or bioterror. Relevant characteristics of toxin weapons will be discussed, as well as limitations to their use. Finally, several natural toxins of bacterial, plant, and marine origin will be evaluated in this context.

Reconnecting with Public Health's Hidden Strength: The Public

The professionalization of public health over the last century has substantially strengthened public health’s science base. But it has also had a serious unintended consequence: disconnecting public health workers from the people they want to protect. As public health professionals concentrate on communicating their expertise to the public – through public education, risk communication, and social marketing campaigns – they are de-emphasizing the importance of listening to and learning from the public. This lack of two-way communication and learning is, in turn, undermining the effectiveness, voice, and power of public health. In this presentation, Dr. Lasker shows public health professionals how they can take advantage of one of their most valuable untapped resources: the experiential knowledge of community residents and their strong interest in contributing that knowledge to protecting and improving health.

Following the September 11 attacks, the blood banking community learned valuable lessons about its role in emergencies. A national task forced was formed in 2002 to make certain that blood collection efforts in response to domestic disasters and acts of terrorism run smoothly and are managed properly, with the public receiving clear and consistent messages regarding the status of America’s blood supply. This presentation will discuss the responsibilities of local blood centers in disasters and examine national efforts to ensure that blood collection facilities maintain safe and adequate inventories at all times in preparation for disasters.

Health Hazards of a Clandestine Methamphetamine Laboratory

Methamphetamine drug labs are a growing public health hazard in Iowa. According to the Iowa Department of Public Health, in 2003, federal, state, and local authorities seized 1,100 Iowa labs, and the number is growing every year. The chemicals involved in the synthesis of methamphetamine are highly toxic not only to the people who produce the drug but also to those who clean up the labs. In this presentation, Ed Bottei, medical director of the Iowa Statewide Poison Control Center, state medical toxicologist for the Iowa Department of Public Health, and clinical assistant professor of pulmonary and critical care medicine at The University of Iowa, will discuss methamphetamine toxicology and ways that cleanup workers can protect themselves.

In Iowa, multiple local and state agencies have worked collaboratively to develop the Biological and Chemical Threat Agent (BCTA) protocol model. In this presentation, three experts in emergency preparedness will discuss the BCTA protocol model and give advice on how local and statewide agencies can develop similar emergency and disaster response protocol models.

Plan, Prepare, and Practice: Elements of Emergency Exercise Design

This program is based on the premise that emergency exercises are worth the effort. Exercises identify areas of competence and those that need improvement. Lessons learned from exercises can be used to revise operational plans and provide a basis for training to improve proficiency in executing those plans. The purpose of the lecture is to introduce participants to fundamentals of emergency exercise design. It will provide guidance on how to plan and conduct a small functional exercise for organizations with emergency response roles. The speaker will cover the purpose, characteristics, and requirements of three main types of exercises—tabletop, functional, and full-scale—as well as discuss exercise evaluation and enhancements.

Influenza Overview Putting the US Flu Season and Asian Bird Flu Outbreak into Perspective

This has been an unusually eventful year for influenza. In the U.S., seasonal activity began much earlier than usual. Attention was focused on the occurrence of severe complications in children, the adequacy of the vaccine supply, and the effectiveness of this year’s vaccine. In Asia, the dramatic spread of a large epizootic of influenza A (H5N1) among poultry (commonly called bird flu or avian influenza) and associated deaths among people has focused attention over the development and control of such phenomena as well as the possibility of the next influenza pandemic. These events, and issues raised, will be reviewed and placed in perspective.

Remembering the Zebras: Tools for Public Health Officials and Veterinarians

Common advice given to new graduate veterinarians used to be: “When you hear hoof beats, think horses not zebras.” However, with the increasing incidence of emerging and exotic zoonotic diseases and the concerns regarding bio-terrorism and agroterrorism, it is very important for veterinarians to consider that the hoof beats they hear may be “zebras.” The Center for Food Security and Public Health (CFSPH) at Iowa State University (ISU) has developed innovative approaches to help prepare the veterinary profession to recognize and respond to emerging and exotic zoonotic diseases, potential bioterrorism pathogens, and potential agroterrorism pathogens. Dr. James Roth will describe the CFSPH’s approach to training and demonstrate some of the materials developed.

Jerry Ostendorf has served as the Readiness and Response Bureau Chief with the Iowa Emergency Management Division since 1987. His primary responsibilities include Homeland Security policies and procedures, Emergency Management Assistance Compact activations, and the Federal response Plan coordination with federal partners. He has served as Chief of Operations and State Coordinating office for the state of Iowa through fifteen Presidential Disaster Declarations and hundreds of governor-declared disasters. Role of Public Health in Emergency Preparedness and Response

On April 16 th Incident Command or Management System Training will be provided by a National Association of County and City Health Officials (NACCHO) instructor. Many agencies at the local, state, and federal levels use the Incident Command System (ICS), which has been revised and improved as the Incident Management System (IMS), to coordinate efforts during an emergency response. The role of public health is now integrated into this training program. ICS/IMS can be used for any type or size of emergency and allows agencies to communicate with each other and to coordinate resources. NACCHO, Centers for Disease Control and Prevention (CDC), Federal Emergency Management Agency (FEMA), and Advanced Practice Centers for Public Health Preparedness have partnered in this endeavor. The one day training enables public health participants to learn about ICS/IMS and the role of public health agencies in incident management and emergency response activities. Core public health readiness competencies developed by the Columbia University Center for Health Policy are used in this program.

Trainers will be given the opportunity to gain insight into issues related to disasters and how they affect the public’s health. The physical and emotional well-being of survivors and responders will be discussed. This session will focus on how disasters emotionally impact individuals and the need to provide crisis intervention. Peter Teahen, Director and founder of the International Mass Fatalities Training Center and co-author of the Mass Fatalities Incident Response Plan, is an Iowa licensed funeral director and will lead this session. He has extensive experience in disaster planning, response and training, and CISM. Mr. Teahen will incorporate his experiences into this interactive session. A combination of lecture, discussions and tabletop exercises will provide trainers with insight on how to prepare for, respond to and recover from disasters involving a large loss of life.

Health Hazards Found in Clandestine Methamphetamine Laboratories

This 20-minute video was created to provide a general awareness about health hazards found in methamphetamine laboratories. In this video, Dr. Ed Bottei, Medical Director of the Iowa Statewide Poison Control Center, State Medical Toxicologist for the Iowa Department of Public Health, and Clinical Assistant Professor of Pulmonary and Critical Care Medicine at The University of Iowa provides an overview of the highly toxic chemicals involved in the synthesis of methamphetamine. He also discusses hazards associated with methamphetamine laboratories, and strategies for cleanup workers and others who might come into contact with laboratories to protect themselves.
