Practicum Proposal Form

The Proposal Form must be completed and all required signatures must be obtained before registration.  The Proposal Form must be approved by the Course Director before beginning any site work on your project.

Forms for Oral Presentation

If you choose the oral presentation option, you will need to take a copy of the Faculty Review Committee Worksheet for each committee member, and the Status of Completion Form with you to your presentation:

Forms for Poster Presentation

If you choose the poster presentation option, you do not need to take any forms with you to the presentation. Members of the Poster Presentation Review Committee will use the following form to evaluate your presentation:

• Status of Completion Form

Course Evaluation Forms

At the end of the Practicum Experience, students and preceptors are asked to complete evaluations:

• Preceptor Evaluation of Student Form
