Nominations for the 2027 Iowa Public Health Heroes Award will open in fall 2026. The award recipients will be recognized at a reception in spring 2027. More information will be announced as details are set.

Each nomination should be accompanied by a nomination letter of no more than three pages, which you can upload via the form below. Multiple signatories may be incorporated into the letter. Evidence should include specific information regarding the accomplishments of the individual relative to the award criteria.
